For this project you will need angular gear transmission of an old car jack. Something like this. Those carjacks belong to Fiat 750.
This is how my friend sorted it out. His rig: Toyota Hilux + Artica. He is the author of this improvement.

Excellent modification. Thanks for the write-up and pictures.
On mine the access hole is not obscured by the camper, but the overhang of the camper means you cannot swing the crank handle. So luckily all I needed to do was create an extension. I think in the end I welded a couple of sockets together back to back and that allowed me to use my socket set on the end of the square drive crank shaft. I say "think" because in 6 years I've never had a puncture, so never had to remove the spare wheel while the camper is on. I might try taking the spare wheel off this afternoon to make sure it hasn't jammed up there, or cracked. In fact I have another spare wheel and tyre in better condition, so I might swap them over.

No new holes were drilled into the chassis. We used those which already existed there.
